The following requirements should be accomplished for admission to the program: 

  1. Completion of Enrollment Agreement 
  2. Payment of Registration Fee 
  3. Must be at least 18 years old 
  4. Must possess a valid government issued ID
  5. Must speak, write and understand English
  6. Must submit proof of 12th grade education or its equivalent: High School Diploma from an accredited school or General Education Diploma (GED). 
  7. * Foreign graduates must have a high school diploma or transcript evaluated for 12th grade equivalency in the US.

  8. Must possess or in the process of obtaining a Social Security Number or TIN at the time of enrollment
  9. Completed medical history, physical examination and PPD signed by a physician or nurse practitioner prior to any direct patient care
  10. Current CPR certification with BLS prior to direct contact with patients
